Targets / mechanism
- alpha-2 adrenergic receptor agonism
- sympathetic outflow reduction
- sedation/somnolence
Optimization domains
- blood pressure
- ADHD
- sleep
- anxiety
- withdrawal
- cardiovascular
Research basis
- Clonidine's alpha-2 agonism explains why it is discussed for adrenergic physical symptoms and sleep initiation.
- The evidence base is much stronger for BP/ADHD-related contexts than for adult insomnia maintenance.
- It is useful in the graph as the oral alpha-2 comparator to dexmedetomidine, not as a clean sleep drug.
Limits, risks, and missing evidence
- Sedation does not guarantee longer or better sleep, and it can cause hypotension, bradycardia, dry mouth, next-day impairment, and rebound hypertension.
- Combining with beta-blockers, sedatives, antihistamines, trazodone, or other antihypertensives can compound hemodynamic and CNS risk.
- Adult chronic-insomnia evidence is weak compared with purpose-built insomnia treatments.
Risk flags
- approved drug
