Research basis
- Sertraline is an approved SSRI with randomized anxiety/depression evidence; in optimization terms it is relevant where anxiety is limiting sleep, training consistency, or function.
- The PANDA trial is useful because anxiety-symptom improvement can precede or exceed depression-score changes in primary-care patients.
- Compared with clomipramine, it is generally a lower-toxicity serotonergic first-line option.
Limits, risks, and missing evidence
- Delayed onset, activation, GI effects, sexual dysfunction, emotional blunting, withdrawal, and serotonin-syndrome interactions matter.
- It is not an acute fix for physical panic symptoms and can initially worsen agitation or sleep in some users.
- Combining with MAOIs, stimulants, serotonergic TCAs, or multiple sedatives raises risk.
